Abstract : 25 hydroxyvitamin D3 is required for the production of insulin as well as improvement of insulin sensitivity. Increased prevalence of gestational diabetes mellitus (GDM) representing 12.5% of all pregnancies and decreased 25(OH)D3 levels are common among Saudi population. This study was designed to explore the 25(OH)D3 status in normal and GDM group, in addition to demonstrate the relationship between 25(OH)D3 levels and insulin resistance in normal pregnancy and in GDM Saudi pregnant women. The study included 103 pregnant women classified into two main groups: normal pregnant (n = 48) and GDM (n = 55) groups. Further classification of each group was carried out based on 25(OH)D3 statuses (deficient, insufficient and sufficient). Serum were used for determination of fasting blood glucose (FBG), fasting insulin(FI), C-peptide, cholesterol, triacylglycerol (TAG), total Ca, 25(OH)D3 and parathyroid hormone (PTH). IR markers were calculated including glucose/insulin ratio (G/I), fasting insulin resistance index (FIRI), homeostasis model assessment of insulin resistance (HOMA-IR), Log HOMA-IR, HOMAA1- %B, HOMA β cell. Result indicated significantly increased mean values of maternal age, BMI, FBG, FI, C-peptide, FIRI, HOMA-IR, Log HOMA-IR, HOMA β cell, cholesterol and TAG in GDM group compared to control group. Mean values of 25(OH)D3 did not show significant variations between GDM and control group. In control group, non significant correlations were obtained between 25(OH)D3 and each tested parameters, however, in GDM 25(OH)D3 showed significant correlations with FBG, Log HOMA-IR and PTH. Results also revealed increased prevalence of 25(OH)D3 deficiency among healthy pregnancy and GDM(~69% and 67% respectively) groups. This study indicated that IR was improved in GDM subjects by improving 25(OH)D3 status. It could be concluded that 25(OH)D3 may be associated with elevated IR that accompany GDM.
